

Peoria Institutional Review Board
It is the policy of UICOM-P IRB that all investigators and staff desiring to engage in research using human subjects be familiar with policies and procedures and related federal regulations.
The principal investigator and all key personnel (i.e., staff, coordinators, sub-investigators involved in any way in the recruitment or interaction of potential or existing research subjects) conducting research involving protected health information (PHI) must successfully pass a test involving the HIPAA privacy rule and protecting health information. The principal investigator and all key personnel must also successfully pass NIH certification and provide a current curriculum vitae. Detailed instructions are provided below:
NOTE: If you are requesting a change in personnel, the Change in Research Form must be completed. All NEW personnel will be taken to the full board for review and approval.
HIPAA
Certification:
1)
Review the UICOMP HIPAA slide presentation.
2. Review the HIPAA Privacy Rule Executive Summary.
3) Review the list of HIPPA Identifiers
4) Download and complete the HIPAA Certification Examination
5) Return test to the OHRO office, ATTN: Brenda Hedges.
NIH Certification:
1) Follow the link to take the test: http://cme.cancer.gov/c01/nih_intro_01.htm
2) Upon completion, you will receive a certificate.
3) Print off the certificate and send to the OHRO office, ATTN: Jessica
Svendsen. Please also provide a copy of your current CV.
